12 Pharmacies in Bilbao City Centre

Finding a reliable pharmacy in an unfamiliar city can be a stressful experience, especially when you need medication urgently or are dealing with a sudden illness. In Bilbao, the dense urban layout of the city centre means you are rarely far from professional healthcare support. Whether you are a tourist navigating the charming streets of Casco Viejo or a resident running daily errands, knowing where to go for pharmaceutical needs is essential.

Bilbao’s city centre, encompassing areas like Indautxu, Abando, and the Old Town, is well-served by a robust network of farmacias. These establishments offer much more than just prescription fulfilment; they are hubs for health advice, over-the-counter remedies, and specialized dermocosmetic products. The high concentration of pharmacies ensures that you can access trusted advice quickly, often without needing to visit a hospital for minor ailments.

Spanish pharmacies are highly regulated and staffed by licensed professionals who can offer clinical guidance. You can identify them by the distinctive green cross sign, which often lights up or flashes when the store is open. In Bilbao, many pharmacists speak English, particularly in the tourist-heavy areas, making it easier for international visitors to explain their symptoms and receive the correct treatment.

The city operates a strict "guardia" (on-call) system to ensure 24/7 access to medication. Even if the central shops are closed for the night or during a public holiday, at least one pharmacy in the area will remain open. Information regarding the current on-call pharmacy is typically posted on the door of every closed location, ensuring you are never left without access to essential medicine.

Beyond emergency care, pharmacies in Bilbao city centre are excellent resources for wellness products. You will find a wide selection of high-quality Spanish and French skincare brands, baby care items, orthopaedic supports, and homeopathic remedies. The staff are usually happy to help you navigate local brands and find equivalents for products you might use in your home country.

Because the city centre is highly walkable, you are usually just a two or three-minute walk from the next pharmacy if one is closed or out of stock. This density is a significant advantage for residents and visitors with mobility issues or those managing chronic conditions that require frequent prescription refills.

Below is a curated list of established pharmacies located in the heart of Bilbao. These locations have been selected for their central accessibility, professional reputation, and service offerings to help you find the care you need quickly.

I'm a resident of this neighborhood, and I've been to this pharmacy maybe a hundred times since I've lived here (over 10 years). I was taking a picture of a dietary supplement (the side of the box with the ingredients) to ask the pharmacist if it was what I needed when the owner rudely and impertinently came over to confront me, saying that no photos were allowed in her pharmacy… a picture of a supplement! I'm sorry, but neither I nor my family will be coming back. Good luck.

I'm from Murcia and was visiting Bilbao when I started feeling a bit unwell. I didn't know whether to go to the emergency room and wait for hours, so I went to a pharmacy in the Old Town. A young man with glasses helped me, and when he noticed my accent, he asked me where I was from in a very friendly and non-intrusive way. When I told him I was from Murcia, he replied with a "posh acho" and mentioned that he had a friend living there. He made me feel very comfortable, and even though my situation was a bit personal, he treated me with great empathy and naturalness. He recommended something that made me feel better very quickly. Thank you for your kindness and attention.

I am a regular customer of this pharmacy, but during my last visit, when I came to buy a feminine health product, the employee asked me twice where I was from: first where I live and then “where I’m originally from.” These questions were unrelated to customer service and violated my sense of privacy. The entire conversation was conducted in Spanish from the beginning, so these questions felt especially odd. At the end, he said goodbye in a language that, according to him, I should understand, which I found inappropriate. According to Regulation (EU) 2016/679 (GDPR) and Organic Law 3/2018 on the Protection of Personal Data and Guarantee of Digital Rights (LOPDGDD), organizations may only collect personal data that is necessary and proportionate to the purpose of the service (Article 5 of the GDPR — principle of data minimization). I found it an uncomfortable situation and now I'm not sure I want to continue shopping at this place, which is a shame because I really like their selection and products.

The service left much to be desired. The employee who helped me was very rude and arrogant; when I asked for help, her only response was to go to another pharmacy. Luckily, at another establishment they treated me kindly and were able to dispense what was listed on my health insurance card without any problems.

The staff are absolutely lovely. Plus, it's open until 10 pm on weekdays and weekends. Although it's a bit of a walk from where I live in Santutxu, and the hill is a bit of a climb, the friendliness of the staff more than makes up for it. They're all wonderful.

I went looking for insoles for joint pain in my foot. I received excellent advice. But while we were talking, she sold me some excellent ones along with some simpler but equally functional ones, the first ones for €21 and the second ones for €45! When I used the €21 ones, I realized they were sufficient, and she told me that since I hadn't kept the receipt—in this digital age where everything is recorded—she wouldn't give me a refund. Basically, she sweet-talked me with the promise of amazing insoles, and then I was stuck with them. Be careful what you buy! And keep your receipts because, although there are plenty of ways to get them back when they want to, otherwise you're stuck with the insoles at the price of trendy shoes. She didn't really want to accept any returns, period, but she was very friendly and a good salesperson. It was outrageous. I might ask about my rights at the consumer protection office.

I've been to that pharmacy several times, but yesterday was the first time I left feeling dissatisfied. There were two girls "working" (they were watching a movie). As soon as I walked in, one went inside and another came out. The girl who came out looked at me as if I owed her something. I asked if they could serve me without my health insurance card, and the other girl started speaking rudely to me, as if I'd been asking for it for ages, or as if I were asking for free medicine. It was completely out of line. I could write down everything we said, but the girl wasn't interested in listening; she just assumed I knew what she wanted to say. At that moment, I was having cramps and had no energy for anything. I had just come from the emergency room, but tomorrow I'm going to file a complaint. That's no way to treat anyone. You don't need to smile to work with the public and be pleasant.
Kerman★★★★★
Excellent service in finding a medication that several pharmacies had told me I wouldn't have. I'd been there before, but this assistance was truly professional and attentive.

Until yesterday, I liked this pharmacy. Yesterday, a rather unfriendly girl served me and wouldn't give me receipts. I bought something for someone else and had to ask for a receipt, and then I bought something for myself and she left without giving me one, even though I waited for her to give it to me. I don't know if they know that it's MANDATORY to give a receipt. Poor service.
Miguel Redondo★☆☆☆☆
It would be good if they learned how to use the REMPa electronic prescription system, and stopped treating you like an idiot. If they need to check and store the prescription, they can receive it in multiple ways. If they want to be extra careful, they already ask for the IDs of both the patient and the person picking it up. But I suppose it's more convenient not to bother. And the fact is, just over an hour later, at the on-call pharmacy, they had no problem dispensing the medication with the same PDF prescription generated by REMPa (which doesn't exactly have few authenticity verification mechanisms).

The worst attitude that can be towards people We came because a friend had a hypertensive crisis. We asked to measure the blood pressure. They kicked everyone except her out into the street in the heat of +41. We argued that we couldn't wait in the pharmacy. Zero humanity or understanding. Response to your comment. It's outrageous how you understate and twist the facts First, there were 4 of us in total, not our friend and 5 more people as you write. I was the first to come to the pharmacy, turned to the pharmacist, and only when I found out that your pharmacy had a blood pressure monitor did I go and pick it up from the museum. What you call providing help is that you measured a person's blood pressure for money and advised them to just rest and drink water when the pressure was 155 over 100 (not you personally, but the woman who works there). The same woman who, when her friend returned to wait at the pharmacy for a taxi (it's true), shouted for her to come out - "let her get up and come out, we helped her." You stood in the doorway to keep her out. You probably thought that if you spoke Spanish, nothing would be understood. The only person who behaved humanely was the man who came out when she returned the second time. But the advice to drink water and rest under such pressure is not help. And during the time we were there, there were no other visitors. We did not interfere with other people visiting your pharmacy.
Mar S★★☆☆☆
In my opinion, the customer service was appalling. The pharmacist kept asking questions and giving her opinion, telling me what I should do in a very unpleasant way, without any idea of how things work in other cities. I even told her that I didn't find her manner professional and that the way she asked questions and expressed her opinion was very rude. I've been to many pharmacies and this has never happened to me before. She told me that the laboratories had run out of my pills and that this could happen at many pharmacies, and that I should go to the health center to ask what other brand I could take. I went to another pharmacy nearby and they ordered my pills, which will arrive around 3 pm, with excellent and professional service. I was extremely shocked by the way the pharmacist asked questions and gave her opinion on everything, and how her opinion seemed to carry more weight than anything else.
Ainhoa Arce Muñoz★☆☆☆☆
Terrible service. I went in to ask about a product, and when I decided not to buy it, the owner told me I was wasting her time. It seems they're bothered by people asking questions before buying. By the way, giving unfounded medical diagnoses doesn't inspire much confidence either. Customer service: fails.

I felt completely uncomfortable. The staff was curt and rude, looking at me with indifference. They refused to give me a cream that every pharmacy has given me because, according to them, my eye was fine. I think I know better than anyone whether it hurts or not; otherwise, I wouldn't have gone there. I won't be back.
Ana Orbea Diez★★★★★
I'm amazed by the negative reviews. Every time I've needed to go, the service has been fine. I don't understand this animosity towards this pharmacy.
Alex Sukharev★☆☆☆☆
This review isn't just about this pharmacy, but about the entire system. Recently, during a workout, I experienced severe chest pain. I had to stop and went to this pharmacy to ask for nitroglycerin or something similar. They refused because this medication (like most others) requires a prescription. They recommended I see a doctor. I want to emphasize that in situations like this, time is of the essence, and without immediate medical attention, a person can die or suffer serious long-term consequences. Afterward, I had blood tests done, and it turned out I had elevated troponin levels: myocardial ischemia was confirmed. The pharmacy staff and the system in general don't care about people's lives or health. You could literally die in their pharmacy, and no one would offer you even the most basic assistance, because for them, following regulations is more important than saving a life. Understand that if I take any medication, it's my responsibility, and I bear the consequences for my body; it's not their business. But when they fail to provide basic assistance and a person dies or is harmed, that is their responsibility, at least morally.
Endika de Diego★☆☆☆☆
I went there after Covid, they wouldn't let me in without a mask, and the only solution they offered was to buy one for €1.20 if I wanted to enter. They weren't outside, very nice. Good business, I hope they did well with all the money they made ripping people off.

Tips for Visiting Pharmacies in Bilbao

Navigating the pharmacy system in a new country can sometimes feel confusing, but a little preparation goes a long way. While the standards in Bilbao are exceptionally high, understanding the local nuances regarding prescriptions, payment, and opening hours will ensure a smooth experience. Here are some practical tips to keep in mind when seeking pharmaceutical care in the city centre.

Keep your prescription or medication box handy. If you are visiting from abroad and need a refill, Spanish pharmacies cannot always dispense foreign prescriptions directly. However, showing the pharmacist your current medication box or a written summary from your doctor can help them determine if an over-the-counter equivalent is available or if you need to visit a local medical centre for a Spanish prescription.

Understand the "Guardia" system. While the pharmacies listed above generally keep standard business hours, you might need medicine late at night. Do not wander the streets looking for an open door. Instead, check the window of any closed pharmacy. They are legally required to display a sign indicating the nearest 24-hour or on-call pharmacy in the district. You can also search online for "farmacia de guardia Bilbao" for real-time results.

Don’t underestimate the pharmacist’s role. In Spain, pharmacists are highly trained healthcare providers. For minor issues like colds, allergies, insect bites, or blisters from walking the city’s hills, you do not need to find a doctor first. Describe your symptoms directly to the pharmacist; they are authorized to dispense appropriate medication and will advise you if a doctor’s visit is necessary.

Be prepared for payment. While large pharmacy chains might accept international credit cards, many independent local pharmacies prefer cash for small purchases or may have a minimum spend for card transactions. It is always wise to carry a small amount of cash (€10-€20) when picking up over-the-counter items, just in case.

Ask about generic brands (EFP). Medication in Spain is often significantly cheaper than in other countries, especially generic brands. If you are buying painkillers or antihistamines, ask the pharmacist for the "genérico" version. It contains the exact same active ingredient but will save you money compared to the heavily advertised brand names.

Whether you are dealing with a minor headache after exploring the Guggenheim or need to restock a vital prescription, the pharmacies in Bilbao city centre offer a lifeline of professional support. The combination of expert staff, strict regulatory oversight, and high density of locations makes managing your health here straightforward and safe. By knowing where to look and how the local system operates, you can spend less time worrying about logistics and more time enjoying the vibrant Basque culture.
