Finding a pharmacy when you need one can quickly turn from a simple errand into a stressful search, especially if you are unfamiliar with the layout of a city or dealing with an unexpected ailment. In a bustling urban hub like San Sebastián, knowing exactly where to go saves you precious time and energy. Whether you are a local resident running daily errands or a visitor navigating the picturesque streets of the Old Town, access to reliable healthcare products is essential.
San Sebastián, known as Donostia in Basque, is a city that seamlessly blends high-end shopping, culinary excellence, and residential life within its compact centre. Because the city is so walkable, pharmacies are generally clustered along the main commercial arteries and pedestrian thoroughfares. However, the sheer density of streets and the volume of tourists can make a specific location difficult to pinpoint without guidance.
This guide focuses specifically on the city centre, encompassing areas like the Parte Vieja (Old Town), the Ensanche, and the streets surrounding the Cathedral. We have curated a list of accessible locations to ensure you can find medical supplies, skincare, or prescription services without wandering too far from your hotel or home. It is important to note that Spanish pharmacies (farmacias) are highly regulated and staffed by licensed professionals who can offer medical advice for minor conditions.
When you are in a rush, you do not want to rely on outdated maps or vague directions. The information provided here aims to be evergreen, highlighting the pharmacies that have established themselves as mainstays in the local community. You will find that many of these establishments offer multilingual assistance, given the international nature of the city.
Beyond just dispensing medication, these pharmacies serve as health hubs, offering services such as blood pressure monitoring and dermo-cosmetic consultations. Understanding the operational nuances, such as the Spanish system of "guardia" (on-call) pharmacies, is vital for travelers. While most stores close for the afternoon siesta or on Sundays, there is always a designated pharmacy open somewhere in the city.
To make your search easier, we have compiled a comprehensive list of locations in the heart of the city. This directory is designed to be your go-to resource, ensuring that health and wellness are always within walking distance during your time in San Sebastián.

I came here to buy a few things since I’m travelling and I was taking my time as they were a few options . One of the guys came and accused me of opening boxing with a sharp object, when I was going to purchase a few things and he had no evidence and told me I told him I am opening things to try. I felt so disrespected and offended I left without buying anything. An absolute shame and horrible way of treating a customer I will never come back to this place

Seems a scam! Never received order and they have ignored many emails and have not refunded. Avoid avoid avoid.

Bad experience. They refused to accept my official prescription for vitamin D, claiming it was for single use, despite it being an ongoing treatment. Every other pharmacy has never given me any trouble; in fact, I went to another one nearby right afterward and they served me immediately without any problems.

They use the worst delivery service, TIPSA, which causes a lot of problems and doesn't deliver orders. I've been waiting for over a week and sending several emails, but this shameful company hasn't responded. Now I'll have to contact my bank. Avoid the hassle and order from another pharmacy! It's the last thing you want if you're feeling unwell.

Highly recommended pharmacy and very friendly staff! Lorena helped us and offered exactly what we needed for our newborn. A wonderful service!
M. Hernández Rivas★☆☆☆☆
My experience at this pharmacy was very disappointing. I've always shopped here and never had any problems, but the service I received from Lorena, the pharmacy technician, was unacceptable. She was rude and dismissive, something that not only I noticed, but also other people waiting in line. Her responses were unprofessional and delivered with an unnecessary air of superiority.
Unfortunately, after this experience, I will not be returning or recommending it to anyone I know.

I went to buy plasters for open wounds because I caught my hand in a cabinet. The young man who served me, dark-haired with light eyes, confidently sold me adhesive tape for sprains on the feet and wrists, telling me I could put it directly on the open wound or on top of the plaster.
It's super-adhesive tape for immobilization, not for wounds. If I had put it on the wound, it would have torn off the skin and caused an infection. I think that's extremely serious.
This is the second time I've received bad advice at this pharmacy in Garbera. I understand that everyone can make mistakes, but with health issues, you can't afford to. Please train your staff better because this could have ended badly.

"The service matters more than the sale." That's how you build customer loyalty.
I received impeccable service and top marks for professionalism, even though the glasses broke.
I want to express my deep gratitude to the team behind this pharmacy, especially to Tatiana for her friendliness, empathy, problem-solving skills, and diligence. And to Pablo, for his attention, kindness, and patience; thank you for advising me and giving me all the time I needed to choose.
I bought a pair of glasses before embarking on a trip, on the afternoon of Friday, March 13th, because I couldn't find mine. Pablo, a young professional, helped me, giving me all the options and selling me the model that suited me best. I left delighted and incredibly grateful. Four days later, on Wednesday the 18th, one of the temples broke despite having worn them only a few times, and I was saddened because, being from a pharmacy, I expected higher quality.
I decided to go in just to explain what had happened, without having the receipt or the case with me, and Tatiana immediately decided to resolve it in the best way: "Give me your phone number and I'll find the receipt to generate a voucher for you." The next day, she called to let me know I had a voucher for the full amount of the glasses: €19.90, and told me to come and pick it up whenever I wanted. I brought the receipt and the case, even though no one asked for it. It was a pleasure to see that the service could be exceptional, even if the glasses weren't "that great."

I don't even know how I'm going back. They're so unpleasant. I asked them for something, and they try to sell you whatever suits their agenda. I went in with a cold sore to ask for herpes treatment, and they wouldn't listen. I told them three times it was herpes, and they just kept saying things like it looked like an irritation, or that it was red... all to try and sell me a repairing cream that costs three times as much as the treatment I need and that won't even work... it's not the first time... I don't recommend them.

Volveré a comprar 100%.

I'm not a regular customer of the pharmacy, but I placed an online order and was amazed at how quickly it arrived, and on top of that, it was full of wonderful samples of creams and face products, which are always great. But the most amazing thing is that I contacted them via WhatsApp with a question, and to my surprise, they not only replied immediately but also suggested several alternatives. I looked into it and had some concerns because of my allergies, etc. Well... instead of ignoring me or suggesting other options, Teresa consulted the medical department and has kept me informed throughout the entire process!
What can I say... they've won me over! Professional, humane, and empathetic service... and I thank them, and they still thank me... anyway, congratulations, Ruiz de Azua Pharmacy! That's the way! Kisses!

It's like a brothel, people waiting and behind the glass she's chattering and laughing. There can't be a manager because they're experts at shirking their duties. I ask for diosmin and they sell me Venoruton, not the generic, at whatever price they want.
In response to your comment: I thanked everyone there and I appreciate everyone who serves me, Saturday, Sunday, or weekday. There's a lack of transparency regarding prices, and I don't think it's right that I can't decide whether I want to spend €14.95, €18.50, or €22 for "the same thing."
I'm speaking as a customer who understands that other customers wouldn't appreciate this treatment, and for that reason, I don't think I should have to reveal my address or what I do for a living—information that was accessed without my consent. Thank you for your excellent confidentiality.

Not recommended.
I had a skin condition and bought a cream prescribed by my doctor. Since I had an upcoming appointment with the dermatologist, I didn't use it in case it wasn't the right one. When I went to the dermatologist, he told me that cream wasn't what I needed and prescribed others.
I went to exchange the cream, still in its box with the seals intact, to buy new ones. I hadn't touched it precisely in case I needed to exchange it.
They told me that once something leaves the pharmacy, nothing can be exchanged.
Naturally, I didn't buy the new creams there, and I never will again.
Not a recommended place. There are other pharmacies where the return policy goes beyond simply leaving the store.
I've seen good reviews, praising the empathy, the helpfulness, the staff who travel from far away to this pharmacy...
In short, I wouldn't go anywhere near it. It's nothing special compared to the rest, nor is it better than any other.
And it has this sales guarantee detail.
The guarantee lasts until you walk out the door, and the empathy and help only last until your money is in their cash register.
And moral of the story: if something happens to you, if you can wait to see a specialist, wait and don't buy anything. You might be wasting your money, even though we generally always buy what our doctor prescribes first. I didn't use it initially because I was afraid it would cause a bad reaction, so I waited, but it was no use. I've thrown away a significant amount of money because this cream isn't covered by public healthcare.

The woman there isn't very friendly. I've been several times, and she always seems unfriendly. Today, I went to get some medication they didn't have in stock. The condition for her to give it to me within two hours was that I had to pay a deposit. If I didn't pay, she wouldn't order it for me, and I couldn't even leave the pharmacy. Very dissatisfied, I went to another pharmacy 200 meters further down the street, and they served me with a smile. They had the drops cheaper, and I didn't have to pay a deposit. (This is medication, not some shady business.) Please don't go back to this place. They're scammers and unpleasant. If you don't want to work with the public, stay home.

They shouldn't dispense medications for occasional use with such short expiration dates; three and a half months is far too little. I find it shows a lack of empathy on their part. If it were a medication for chronic use, I'd understand, but for occasional use, they shouldn't do this. Look out for the patient. You know that antihistamines are not medications for continuous, chronic use.

We're very happy with this pharmacy because, unlike other pharmacies in the city, whenever we've gone during on-call hours, they've always had the medication we needed for our children so we could take it with us right away. That's why, even though it's not the closest to our house, we'll definitely be back.

I'm on a treatment from the pain management unit, one of the pills is a sleeping pill, and they say it's expired. I tell them it's a treatment I've been taking for almost 9 years and that I'll pay for it in full and come back tomorrow with the box and the active prescription. They say they can't give it to me. I understand that to some extent; she's doing her job. But I don't understand why people who are on medication have to have it constantly reactivated. Today they're leaving me without my sleeping pill, which isn't a big deal, but I think this is terrible for people on regular medication.
Practical Tips for Visiting a Pharmacy in San Sebastián
Navigating the pharmaceutical system in a foreign country can sometimes feel daunting, even for seasoned travelers. While the staff in San Sebastián are generally welcoming and professional, understanding the local customs and operational rules will make your visit significantly smoother. Whether you need a refill or an emergency purchase, these insights will help you communicate your needs effectively and avoid common pitfalls.
1. Look for the Green Cross
Unlike in some countries where pharmacies are located inside supermarkets or general stores, Spanish pharmacies are standalone shops identified by a distinct green cross, often animated or illuminated. When walking through the city centre, keep an eye out for this symbol. It is usually displayed prominently above the storefront, making it visible from a distance even on crowded streets.
2. Understand the "Farmacia de Guardia" System
Pharmacies in Spain operate on a rotating schedule for nights, Sundays, and public holidays. If you need medication outside of standard hours (typically 9:30 AM to 1:30 PM and 4:30 PM to 8:00 PM), you must locate the "Farmacia de Guardia." Every pharmacy is required to post a sign on their door or window listing the nearest on-duty location for that specific day.
3. Distinguish Between Prescription and OTC Needs
While pharmacists in Spain can dispense many over-the-counter (OTC) medications that might require a prescription elsewhere, they are strict regarding antibiotics and controlled substances. Do not expect to purchase antibiotics without a valid prescription from a Spanish doctor. For minor ailments like headaches or colds, however, the pharmacist is empowered to recommend and sell appropriate medication directly.
4. Bring Your European Health Insurance Card (EHIC) or Travel Insurance
If you are an EU citizen, carrying your EHIC is crucial if you need to fill a prescription from a local doctor. While you will still likely pay a percentage of the cost, it reduces the price significantly. For non-EU visitors, ensure you keep receipts for any purchases to claim back through your travel insurance provider, as pharmacy costs are rarely covered upfront by foreign insurers.
5. Ask for Generic Brands (Medicamentos Genéricos)
When purchasing medication, do not hesitate to ask the pharmacist if a "genérico" is available. Spain has a robust market for generic drugs, which are significantly cheaper than brand-name equivalents. Pharmacists are legally required to offer the cheapest option available, but being proactive about asking can help you save money on your healthcare expenses.
6. Pharmacies are Not Convenience Stores
While you might find some toiletries and baby food, do not expect the wide array of snacks, drinks, or general merchandise found in pharmacies in the US or UK.
